Intake Form:

• After scheduling your appointment, you will receive a link to an INTAKE FORM from Elation Health.
• The INTAKE FORM must be completed within 24 hours of receiving the link.

Pre-Consultation Phone Call:

• We will call you to discuss the test and its requirements and to review the INTAKE FORM.

Deposit:

• A $50 deposit is required to secure the appointment.
• The remaining $475 balance will be due on exam day.
• If you are a no-show for the appointment, the $50 is nonrefundable.
