• We are selective about the patients we accept to ensure each individual aligns with our wellness approach.

Intake Form and Pre-Consultation Phone Call:

• Upon scheduling your appointment, you will receive a link to an INTAKE FORM from
  Elation Health.
• You will receive a call from us to determine if we a good fit for you and your wellness
  needs.
• The INTAKE FORM must be completed within 48 hours of receiving the link or else the
  appointment will be canceled.

Consultation Fee:

• There is a non-refundable $350.00 consultation fee.
• This fee will be applied towards any services you initiate with us.
